Have been diagnosed with Chronic Kidney Disease (CKD)
May or may not have Type 2 Diabetes
Are receiving care for their kidney condition
Are interested in participating in clinical research
Final eligibility will be determined by the study team during screening.

Are You at Increased Risk for Heart Failure?
Answer a few quick questions to see if this study may be right for you.
You May Qualify If:
- You are 40 years of age or older
- You have been diagnosed with Type 2 Diabetes
- You have High Blood Pressure
- You have a history of Heart Disease or Cardiovascular Disease
- You have not been diagnosed with symptomatic heart failure
You May Not Qualify If:
- You are under 40 years of age
- You have Type 1 Diabetes
- You have already been diagnosed with symptomatic heart failure
- Your medical history does not meet the study requirements

What Is Chronic Kidney Disease (CKD)?
Chronic Kidney Disease (CKD) is a long-term condition that gradually reduces your kidneys’ ability to filter waste and excess fluid from your blood.
Many people experience few or no symptoms during the early stages, which is why CKD is often called a “silent disease.”
Over time, kidney function may continue to decline, increasing the risk of serious health complications.
Clinical research is helping researchers better understand CKD and explore future treatment options for people living with kidney disease.
